Just as an update, I've identified the individual who I believe has caused this, and further investigation is ongoing in that regard.
I have also now been able to work through all of the changes that were made to the Bungee itself and think I've repaired any damage that was caused. I'm currently working through updating plugins and the bungee itself before that comes back online.
The Minecraft servers themselves are going to require a bit more work as some were seriously damaged as a result of this incident, I'm hoping to get some basic functionality up and running later today.

Unfortunately this evening there was a catastrophic failure of the permissions system, leading to severe network-wide consequences. Due to the complicated nature of remedying this problem combined with Ryan's busy schedule this weekend, the entire Minecraft network (freedom-01, skyblock-01, hub-03 etc) will be offline for at least 24 hours until the issues are properly fixed.

For most cases, yes, it's simpler, I agree. When ban requests were public, the vast majority of them were open and close cases, where there were multiple punishment logs from admins, or screenshots/videos of them blatantly trying to crash the server or whatever.
But it's less simple for the more potentially controversial cases. Those that involve members of the community who didn't start placing minecarts/griefing every time they joined. People should have a chance to see the evidence against them and defend themselves.
Now, I don't blame the admin who added them to the application nor do I blame those who vouched. It was a mistake, mistakes happen. I've made them myself. But within the chamber of admins, those mistakes are less likely to be picked up on. Ban requests have been approved in the admin section without any votes whatsoever, or with just a mere handful of simple vouches.
Sure, when someone gets caught on video placing hundreds of minecarts, we don't need 10 extra people commenting "vouch". But one size does not fit all, and we not only may need the devil's advocate but the player who the request is for should be able to see the evidence against them.

I get the idea behind this.. but I'd rather there not be the risk of forgetting you had /o enabled and type something like "/o his IP is: 1.2.3.4" etc. Would rather have it stick to admin chat for safety.

I've been on the wrong end of items that crash your game immediately simply by hovering over them in an inventory menu. This would make something like that worse. Object.

From what I can gather from the questions and other things I can read on the plugin page, it's main use case is to buy ranks on the forum, presumably either as a pure cosmetic or to get extra forum features like extra boards or whatever. Either that, or the rewards bought can be claimed in whatever video game the forum is for, etc.
And yknow.. that just ain't us. If you join this forum, just like if you join this server, you should have the same access, excluding staff perms, as any other regular user. I accept we have the coin system and shop system, which I'm not entirely a fan of but the fact it's a little incentive to vote redeems that for me.
Anyway, yeah, object. This doesn't fit with our theme at all and doesn't benefit the forum in any way.

2. What did they do?
Not only are they a serial griefer, but I caught them today building some sort of lag machine and filling it with 154 minecarts before I unvanished and banned them.
3. What indefinitely bannable offense does this fall under according to the guidance for sanctions?

The player first rejoined the server not long after reminiscing about being a serial griefer.
His first session did not last long.
He then tried the same thing again just days later.And then again, today. Two bans inside 3 days for trying to crash the server.
5. Are they known under any other names or IPs, and if so, are they currently (indefinitely) banned under those names and IPs? This information is used to keep track of ban bypassers.No
